45-49 Health Assessments
Patients aged between 45 and 49 years old, who are registered in the database, are eligible for a health assessment. The Australian Federal Government supports general practices in offering a one-time, free health check for individuals in this age group who may already have one or more known risk factors for developing chronic health conditions like diabetes or heart disease.
We provide a thorough and all-encompassing health assessment service that evaluates a patient’s physical, mental, and social well-being. This assessment is particularly beneficial for patients with complex care needs, as it offers a more comprehensive understanding of their overall health.
The primary objective of this health check is to identify and prevent the onset of chronic diseases, while also initiating early intervention strategies when necessary. By detecting potential health issues early on, we can implement appropriate measures to support optimal health and well-being.

75 + Health Assessments
At Erindale Medical & Health, patients who are over the age of 75 years are offered the opportunity to have an annual Health Assessment to help maintain good health. The health assessment is an opportunity for your doctor to complete a thorough check-up of your medical, physical, psychological and social health in a structured way.
This is also an opportunity for you to raise any concerns that you may have relating to your health with your doctor and nursing team. The knowledgeable nursing staff can advise and guide you in obtaining home care services to assist you in staying in your own home for longer. For example, Meals on Wheels, and having your house fitted for handrails.
This type of appointment is offered yearly, but should the need arise for assessment sooner, please discuss with either a nurse or doctor.
